Tiroides
La glándula tiroides es una glándula endocrina ubicada en la parte anterior del cuello, tiene forma de mariposa; su trabajo es formar hormonas tiroideas, llevarlas al torrente sanguíneo para que ahí puedan ser distribuidas a todos los tejidos. Las hormonas tiroideas ayudan al cuerpo a utilizar energía, mantener la temperatura corporal y a que el cerebro, el corazón, los músculos y otros órganos funcionen normalmente.
Definición
El cáncer de tiroides es un tipo de cáncer que se origina en la glándula tiroides. El cáncer papilar de tiroides deriva del epitelio folicular tiroideo y es la neoplasia maligna más frecuente en tiroides (85% de todos los cánceres de tiroides).
Epidemiología
Se estima que este año se diagnostican cáncer de tiroides a 52,070 adultos (14,260 hombres y 37,810 mujeres) en los Estados Unidos. El cáncer de tiroides es el sexto cáncer más frecuente en las mujeres. Es el cáncer más frecuente en mujeres de 20 a 34 años. El 2 % de los casos, aproximadamente, ocurre en niños y adolescentes.
Hasta hace poco, el cáncer de tiroides fue el diagnóstico de cáncer que aumentó más rápidamente en los Estados Unidos. Los investigadores creen que parte del motivo de este aumento fue que las pruebas de diagnóstico nuevas y altamente sensibles llevaron a una mayor detección de cánceres más pequeños. El aumento ahora ha disminuido desde el 7 % anual durante la década del 2000 hasta el 1.5 % anual desde 2011 hasta 2015.
Las mujeres tienen 3 veces más probabilidades de tener cáncer de tiroides que los hombres, pero las tasas de mortalidad entre las mujeres y los hombres son similares. Esto sugiere que los hombres tienen un peor pronóstico que las mujeres cuando hay un diagnóstico de cáncer de tiroides. En términos generales, la tasa de supervivencia a 5 años para las personas con cáncer de tiroides es del 98 %.
Clasificación
Los carcinomas de tiroides en general se clasifican de acuerdo a tanto a su nivel de diferenciación como a las células de las que son originarias.
Epitelio folicular tiroideo
-Carcinoma papilar (> 85% de los casos)
-Carcinoma folicular (5-15% de los casos)
-Carcinoma anaplásico (indiferenciado, <5% de los casos)
Epitelio parafolicular tiroideo o células C
-Carcinoma medular (5% de los casos)
Por otra parte el carcinoma papilar presenta más de una docena de variantes histológicas en las que las principales que se destacan son:
Variante folicular: es la más frecuente, presenta núcleos característicos de carcinoma papilar, pero tiene una arquitectura casi completamente folicular.
Variante de células altas: células cilíndricas altas con citoplasma muy eosinófilo que tapizan las estructuras papilares.
Variante esclerosante difusa: crecimiento papilar prominente entremezclado con zonas sólidas que contienen nidos de metaplasia escamosa.
Microcarcinoma papilar: carcinoma papilar, por lo demás convencional,inferior a 1 cm de tamaño.
Clínica
La mayoría de carcinomas suelen ser nódulos tiroideos asintomáticos, aunque la primera manifestación suele ser una masa ganglionar linfática cervical. Estos nódulos suelen ser únicos, siguen los movimientos de la deglución y son indistinguibles de los nódulos benignos a la exploración.
La ronquera, Disfagia, tos o disnea indican un estadio avanzado y suele estar más asociado a los carcinomas anaplásico y medular.
El la velocidad de crecimiento suele variar dependiendo si el carcinoma es bien diferenciado o no, esto dándonos que el carcinoma diferenciado tiene un crecimiento lento mientras que los indiferenciados suelen crecer a una velocidad mayor, destacando el carcinoma Anaplásico en gran medida sobre todos los demás y también destaca por su peor respuesta a tratamiento.
También se debe mencionar que ciertos carcinomas como el Medular pueden llevar a síndromes paraneoplásicos por la secreción de alguna hormona como el polipéptido intestinal vasoactivo o la hormona adrenocorticotropa.
Diagnóstico
Las pruebas diagnósticas que podemos utilizar son:
Ultrasonografía Tiroidea para poder percibir la presencia de calcificaciones, bordes irregulares, si la masa es quística, la hipervascularidad y la presencia de una linfadenopatía regional.
La Gammagrafía tiroidea para encontrar la presencia de la un nódulo frío o caliente, para apoyar en clasificar si es benigno o maligno.
La determinación de tirotropina sérica(TSH), para determinar la función de el carcinoma.
Biopsia por aspiración con aguja fina para observar la diferenciación celular presente y ciertas características celulares mencionadas en el apartado histopatológico.
Tratamiento
Cirugía: tratamiento primario de todas las formas de cáncer de tiroides.
Terapia con yodo radiactivo (se conoce también como terapia con I-131).
